Two Point Hospital Off The Grid DLC Test – Intro
Hi there, this is the Zap. In this review, I introduce the Two Point Hospital DLC Off the Grid and I show you what’s in it, how it plays and in the end, there’s a rating from me. But most of all I want to give you in this review all the information you need to decide for yourself if a purchase is worth your time and money or not.
I have kindly received a free trial sample from Sega UK, thank you very much for this. But this has no influence on my rating, I try to write all reviews as if I had bought the game or DLC myself.
TPH and DLCs in general
Two Point Hospital is the very excellent successor of Theme Hospital. A new installment that really has it all together. It also retains the humorous essence of the original 1997 version and takes it further. If you don’t know the game yet or want to know more about the game in general, I recommend my Two Point Hospital Tips and Tricks articles to you.
With Off The Grid the 4th expansion for Two Point Hospital has been released. The first 3 DLCs and the first Itempack I also presented in a review, you can find the link to here.
Off The Grid comes with new content for the main game, like 3 additional zones and missions, 35 new diseases, 9 of them with new graphics and animations, 4 new treatment rooms and 31 additional items to build in the hospital.
All missions, diseases, and objects are oriented towards the big topic of ecology, alternative and environmentally friendly living and eco-technology.
As with most DLCs, unfortunately, many of the contents are only directly usable in the areas of the DLCs or in sandbox mode. This means that the new diseases and treatment rooms will not appear in the missions of the main game.
I think that’s a great pity, but sadly that’s how it is done in many games. Probably they don’t want to disturb the balancing of the main missions. I’d rather have a switch for that, but there isn’t one here. But I wanted to mention it in any case.
The new furnishings, however, are available everywhere. So at least you have these things permanently available for free use throughout the game.
As usual, the DLC also contains numerous new presenter sayings for Radio Two Point. Besides the usual funny sayings, this time there is a little radio play story as a special feature. It’s now not stunningly complex or exciting. But I found the idea quite funny.
New in Off the Grid: Illnesses
Then I’ll tell you a little bit in detail what’s in the DLC.
First of all, there are now all kinds of new clothes and hairstyles for the patients in the new areas. Batik shirts, Cornrolls hairstyles and all sorts of other allusions to the hippie and eco-scene will be seen in the appearance of the hospital visitors alone.
The new diseases such as Harebrained, where people have got a rabbit head, or Root Snoot, which looks exactly like a turnip face, are nicely drawn and partly linked with special, matching animations. Graphically, everything is on the usual high Two Point level.
Some of the new diseases have great names, such as Woodworms, Retrogaze, Chocolate Shorts, Tenderbox, Flappy Hamper, Carbon Footprint, Eco-Worrier, Root Snoot, Green Fingers or Mage Fright. And of course, there is always a funny little story about where the disease comes from and how it is cured. But I’m not spoiling that here.
New in Off the Grid: treatments
The 4 new rooms are also visually different. The herb garden stands out in particular. Because this new room is based on the ward and works very similarly. Only in this somewhat quirky eco-implementation the patients are not “bedded” in beds but in flowers and tendrils, where they can cure their illnesses wrapped in plants. Of course, as with the ward and fracture ward, the number of nurses can be increased so that there is no traffic jam in front of the herb garden.
Farmacology also offers a very naturalistic treatment, in that the patients suffering from a scarecrow epidemic are put into a corp-out that looks like a combine harvester with an attached farm.
In the woodwork, the Pinocchio-like woodworm sufferers are again thrown into a shark tank.
Somewhat different is the last stop because in the third level high tech is the order of the day and the worst high-tech illnesses can only be cured by tech support. These Glitchy Patch patients all wear turtlenecks and horn-rimmed glasses, the allusion to Steve Jobs is pretty clear. And in the treatment, these future sick people are then led into a white cube where they are whirled around with wild RGB light flashing.
New in Off the Grid: New Items
Exceptionally different are the new food dispensers. Here you can set up raised beds for carrots, lemons, and tomatoes, which eject ecologically valuable vegan food with a coin slot. At the same time, there is a rainwater collector as a replacement for the water dispenser.
This is where an interesting mechanism comes into play. Because the new machines have a filling level that is not simply filled up by caretakers, but which slowly fills itself, i.e. grows. Here you first have to find out how many beds you need for how many patients in order to feed them all.
My special highlight of new items is one of the rewards from the first mission, a doormat marked “Go away”. But I also liked the new butter churn or the 3D illusion glasses as entertainment devices, and the decorative items Dimension gate and Dream catcher.

New maps and missions: Wanderoff County Park
The new missions in the eco-region follow as a short summary. In the corresponding article on my website, I will also list a table with the individual star requirements for the individual maps, if you are interested.
Map 1 is named Wanderoff and is a typical summer campsite. Here we don’t set up our treatments in buildings, but actually in the gaps between the tents and other agricultural things that are standing around.
This means there are no outer walls in this map and as another special feature the complete area of the mission is directly unlocked for building. This saves us from having to buy land. On the other hand, we also have problems because it’s very warm everywhere, and we have to provide cooling and decoration for the whole area.
And it’s also urgently necessary. Because in this first mission there is a so-called Wellness Index. This is a value that is constantly recalculated by Holistix, based on the satisfaction of our patients with our hospital. And depending on how happy the people are, we get more money for each examination and treatment or just less. The prices can, therefore, vary by +/-50%. So we better make sure that everybody is totally happy so that the cash register is right.
Off The Grid DLC Map 2 – Old Newpoint
In this second map in Old Newpoint we have to convert an old farm into a hospital. Besides very small buildings all normal vending machines are blocked for us here. So there is no vending machine food and also the normal water dispensers, candy machines, etc. are all deactivated. Instead, we have to grow carrots and collect rainwater.
We get from the beginning in this card the new treatment herb garden directly without research and also the also new treatment Farmacology follows quickly afterward.
The requirements are mainly about healing people with the two new treatments and feeding the visitors of the hospital with carrots and tomatoes instead of fast food, sweets, and soft drinks.
Two Point Hospital Off The Grid Map 3 – Windsock City
Mayor Tabitha Windsock is concerned that no one has yet named a town after her. And that’s why she wants to found Windsock City, a modern high-tech city that bears her name.
Independence and self-sufficiency with environmentally friendly energy is the topic here. Since we are to supply ourselves off the grid, i.e. away from the normal power grid, it is also our task to produce the power supply for each room with green electricity ourselves.
These eco-generators are already on the map and you can buy them like normal plots. There is wind energy, solar energy and also hydroelectric power plants. Depending on their size they produce electricity for 3 to 25 rooms. The whole thing is a bit different now than in the normal game, but it doesn’t offer a real challenge.
As a new room, the Tech Support is now also added. This room is only available in this one level or in sandbox mode. I can only repeat myself, what a pity that many of the new things from the DLCs are not available everywhere.

So, for a basic price of €8,99, you get here quite a few hours of new gameplay. There are a lot of funny and also a few less funny gags and gimmicks that enhance the game.
Opinion, Conclusion and Rating – with Numbers
With normal game speed, it should be at least 10-15 hours for the three missions. If you like sandbox games, the value of the content increases again.
All in all, Off The Grid, is not quite as good as my favorite DLC Pebberly Island in terms of quality and challenge. But I think with almost 9 Euro there’s still enough money in there that a real fan of the series can strike without hesitation.
Therefore I would like to give the Two Point Hospital Off The Grid DLC a rating of 74 % if you pay the full price for it. And if the price is lower, the rating increases, roughly speaking, with every Euro less, by at least one percent. Just now for the release, there is an introductory discount of 10 % on Steam.
